About the Opportunity

MonashLink Community Health Service is looking for an experienced and dedicated Community Health Nurse to join their multi-skilled and supportive team in Clayton.

The primary aim of this key role is to work with the full range of allied health staff to provide high quality preventative, rehabilitative and curative nursing and health education services to individuals, families and groups. You will focus on the assessment and early intervention and management strategies for those at risk or identified as at the early stages of chronic conditions.

MonashLink Community Health Service is looking for a Registered Nurse (Division 1 in Victoria) with experience working with patients with chronic disease risks (e.g. diabetes, cardiovascular, respiratory disease).
